Cain, 29, was examined by Dr. James Andrews and Dr. David Altchek, both of whom confirmed surgery is necessary. If Cain has it right away, the procedure would end his season but have him ready in time for spring training.
Doctors confirmed Cain does not have any ligament damage in his elbow, which is great, but surgery to remove bone chips is still a big deal. The Giants come into Friday a half-game up on a wild-card spot and still in need of starting pitching despite adding Jake Peavy before the trade deadline.
Cain was placed on the disabled list July 21, retroactive to July 11, due to ongoing elbow issues. Surely in part because of that lingering pain, he's struggled this season: 4.18 ERA (82 ERA+), 2.19 K/BB ratio in 90 1/3 innings of work.
Cain's in the fifth year of an eight-year, $139.75 million deal that also includes a club option for 2018.
